Transition Services
We work with individuals who
want to transition out of the skilled nursing facilities and youth transition from school to adult life. |
Nursing Home TransitionNWCCI Transition program explores options and provides assistance for community living. NWCCI offers life skills training, peer support, and advocacy. Our staff will assist in getting necessary supports in place including but not limited to home health care, doctor appointments, equipment, applications and any other assistance to reduce risks and allow for successful transition back into the community. Our staff will also assist families and care teams with locating resources to provide assistance.
Youth TransitionTransitioning from school to adult life is a huge step for any youth. NWCCI staff work with the youth and their families to plan and prepare for this transition and to help make the transition as successful as possible. NWCCI staff can provide support to help youth gain skills and move forward with job-seeking, higher education, access to benefits and/or other vocational activities.

TransportationNWCCI offers medical, nonmedical transportation services, and transportation options.
Peer GroupsIndependent Living is rooted in a peer support model. No one could be better equipped to understand and support an individual with disabilities than another individual who similarly experiences life. Most staff working at NWCCI and our Board of Directors have disabilities themselves or may have family members who have disabilities. Which means we all have a shared experience of disability.
We at NWCCI are a part of peer groups that offer peer-to-peer support, guidance, and fellowship. If you would like to get involved in one of our peer groups or would like to start a group that is not formed in our community, please call us to get started. Benefit AssistanceNWCCI can assist with navigating the variety of benefits systems including Medicaid and Social Security.
